How it works:

1. Refer as many people as you like!
2. Earn $75 for each referral.
3. Each person you refer also gets $75.

By participating in the Program, a Referrer represents that he or she has her friends’ prior consent to provide their contact information.

Qualified Referrals: A Qualified Referral occurs when:

• A Referrer is a current patient at one of our offices
• The referred pateint visits the office for a complete exam and cleaning

Reward can be redeemed either a gift card or equal dollars towards treatment on your account. Rewards are not transferable between any individuals
