Qualified Referrals: A Qualified Referral occurs when:
- A Referrer is a current patient at one of our offices
- The referred patient visits the office for a complete exam and cleaning
Reward can be redeemed either a gift card or equal dollars towards treatment on your account. Rewards are not transferable between any individuals
